A qualitative study on the post-traumatic growth course for women with termination of pregnancy due to fetal abnormalities

Objective To explore the post-traumatic growth course for women with termination of pregnancy due to fetal abnormalities and analyze the promoting factors,in order to provide a basis for nursing and intervention tactics. Methods The purposive sampling and theoretical sampling methods were used to select 16 women who were diagnosed with fetal abnormalities and decided to terminate pregnancy in the obstetrics department of a tertiary care maternity hospital in Beijing from May 2021 to February 2022. Women were interviewed by semi-structured interviews,and the data was analyzed by Charmaz’s constructing grounded theory paradigm. Results Women with termination of pregnancy due to fetal abnormalities underwent 4 stages of post-traumatic growth:traumatic stress period,rumination and seeking of help period,and acceptance internalization period,and post-traumatic growth period. The behavior of each period was influenced by the psychological experience of the corresponding period and influenced the experience of the next period. Extroverted personality,previous normal pregnancy and delivery experience,positive perception of events,positive self-identification,good social support,appropriate hospitalization environment and regulations,open mindedness towards death and religious belief,affected women’s post-traumatic growth. Conclusion The post-traumatic growth course of women with termination of pregnancy due to fetal anomaly is a complex continuously sublimated process. Medical staff should pay attention to psychological and behavioral changes in women’s post-traumatic growth,combine the factors that promote post-traumatic growth,and provide targeted guidance to help them successfully go through this difficult period and achieve personal growth.

Evidence summary for insertion and maintenance of mini-midline

Objective To search,evaluate,and summarize the best evidence of insertion and maintenance of mini-midline in adult patients,in order to provide a solid evidence-based foundation for establishing clinical standardized and safe utilization of mini-midline. Methods We systematically searched UpToDate,BMJ Best Practice,Guideline International Network,National Guideline Clearinghouse,Joanna Briggs Institute,National Institute for Health and Care Excellence,Centers for Disease Control and Prevention,Scottish Intercollegiate Guidelines Network,Canadian Medical Association:Clinical Practice Guideline,Registered Nurses’ Association of Ontario,Intravenous Nurses Society,Cochrane Library,PubMed,CINAHL,Embase,Web of Science,Yimaitong,China National Knowledge Infrastructure,Wanfang Database,and China Biomedical Literature Database. It includes clinical practice guidelines,expert consensuses,evidence summaries,systematic reviews,meta-analyses,and best practice on mini-midline insertion and maintenance. The search period covered January 2013 to May 2023. 2 researchers independently evaluated the literature quality and extracted the literature that met the standards. The evidence was graded using the JBI evidence pre-grading system(2014),and the extracted evidence was classified and summarized by themes. Results A total of 10 pieces of the literature were involved,including 6 guidelines,2 expert consensuses,and 2 systematic reviews. This study summarized 20 pieces of best evidence in relation to the following 4 themes:indications and contraindications for mini-midline,catheter insertion,catheter maintenance and catheter removal. Conclusion The study comprehensively and scientifically summarizes the best evidence regarding the insertion and maintenance of mini-midline. We recommend that clinical practitioners integrate this evidence into their practice,while considering individual patient preferences and medical contexts. This approach,in line with the principle of individualization,will contribute to enhancing the standardization and safety of mini-midline use in clinical settings.

The current situation and influencing factors of patient perception for humanistic care in 30 provincial hospitals

Objective To understand the current status and influencing factors of patient perception for humanistic care in China hospitals,and to provide a basis for developing nursing humanistic care measures and improving the quality of nursing humanistic care services. Methods A total of 30,099 outpatients and inpatients from 107 hospitals in 30 provinces(autonomous regions and municipalities) from July to August 2022 as survey subjects. A general information questionnaire and the Relational Caring Questionnaire-Patient Form were used for a cross-sectional survey,and a single-factor analysis was used to analyze the influencing factors of patient relationship care. Results Finally,29 108 valid questionnaires were collected,and the effective questionnaire recovery rate was 96.7%. The patient evaluation of relationship care was(65.72±8.61) points. Single-factor analysis showed that gender,age,marital status,children’s situation,education level,occupation,place of residence,average family income,medical insurance type,visiting department,and location of the visiting hospital,and whether or not surgery were influencing factors of patient relationship care(P<0.05). Conclusion The evaluation score of caregiver-patient relationship care among Chinese hospital patients is above average,but there is still room for improvement in western and rural regions,seriously ill and outpatient patients,low-income and low-medical insurance reimbursement populations,and non-surgical patients. Medical institutions at all levels should optimize and improve nursing humanistic care services based on influencing factors,and further enhance patients’ perception of nursing humanistic care.

Intervention study of lung cancer patients undergoing postoperative chemotherapy based on sentinel symptoms

Objective Based on sentinel symptoms,a nursing intervention program for gastrointestinal symptom group of postoperative chemotherapy for lung cancer was constructed and its application effect was evaluated. Methods The nursing intervention program of gastrointestinal symptom group was constructed on the basis of reference guidelines,qualitative interview and expert consultation. From January 2021 to January 2022,a total of 330 patients with postoperative chemotherapy for lung cancer in a tertiary hospital in Shenyang were selected as research subjects. The experimental group received the gastrointestinal symptoms group nursing intervention program on the basis of routine nursing,and the control group received routine care. Patients were investigated with the M.D. Anderson Symptom Inventory and the MOS 36-Item Short-Form Health Survey before 1st chemotherapy(T1),3rd chemotherapy(T2) and 5th chemotherapy(T3). Results After the intervention,the total scores of the 2 groups and the total scores of each symptom in T2 and T3 were statistically significant(P<0.05),and the score of the experimental group was lower than that of the control group. For the scores of 6 dimensions of physiological function,physical pain,overall health,vitality,emotional function,mental health in 2 groups between different time points,the differences are statistically significant(P<0.05). Conclusion The nursing intervention program of the gastrointestinal symptom group based on sentinel symptoms is beneficial to reduce the severity of the gastrointestinal symptom group and improve the quality of life for postoperative chemotherapy for lung cancer patients.

Development of a knowledge-attitude-practice assessment questionnaire for the investigation of home bathing status of ostomy patients and the test of its reliability and validity

Objective To develop a knowledge-attitude-practice assessment questionnaire for home bathing status of ostomy patients and to test its reliability and validity. Methods Based on the theory of knowledge,attitude and practice,the initial questionnaire was generated through literature review and group meetings. The final questionnaire was formulated by 2 rounds of Delphi expert consultations and pilot-testing. A convenience sample of 386 stoma patients who visited the stoma outpatient clinic of a tertiary hospital in Zhejiang Province from July to December 2022 were selected to examine the reliability and validity of the questionnaire. Results The final knowledge-attitude-practice assessment questionnaire included 3 dimensions of knowledge,attitude and behavior,with a total of 24 items. The CVI of each item in the questionnaire ranged from 0.800 to 1.000,with an average of 0.932. Exploratory factor analysis showed that cumulative contribution rate was 67.181%. Confirmatory factor analysis showed that the assessment questionnaire had a good fit:the chi-square degree of freedom ratio(χ2/df)was 1.510;the fitting goodness index(GFI) was 0.878;the comparative goodness of fit index(CFI)was 0.948;the root means square error of approximation(RMSEA) was 0.049. The total Cronbach’s α coefficient of the questionnaire was 0.922,and the Cronbach’s alpha coefficient of each dimension was 0.920,0.862,0.897;the test-retest reliability coefficient of each dimension was 0.856,0.942,0.926. Conclusion The knowledge-attitude-practice assessment questionnaire has good reliability and validity,and it can be used as a valid tool for medical staff to carry out assessment and education of home bathing for patients with stoma.

Epidermolysis bullosa acquisita is a rare autoimmune blistering disease with bullae formation on the skin and erosions on the mucous membranes. Integrated wound management promotes wound healing,reduces patient discomfort,and improves patient quality of life through medication care,wound care,injury prevention,nutritional support,and comorbidity care.This paper reviewed the staging of epidermolysis bullosa acquisita,wound assessment tools,factors affecting wound healing,and integrated wound management methods,with the aim of providing a reference for improving related research and improving the quality of nursing practice in China.

Objective We aimed to establish an evaluation index system for health education among patients with nasopharyngeal carcinoma(NPC) who are undergoing radio-chemotherapy. As a theoretical basis,this system may be applied by the clinical nurses to carry out targeted health education and guidance among NPC patients undergoing radio-chemotherapy and to evaluate the effect of health education. Methods From March to July 2021,we constituted a structured multidisciplinary research team,including oncologists,nurse specialists,nutritionists,et al,based on the theory of Knowledge-Belief-Practice. Literature review,semi-structured interviews,as well as the Delphi method and Multi-dimensional Analytic Hierarchy Process,were used to establish a framework of the evaluation index system and set weighted value for each evaluation index. Results The response rates of the 2 rounds of expert consultations are 85.0% and 100%.The expert authority coefficients are 0.932 and 0.935,respectively.The total Kendall harmony coefficients of the 2 rounds of expert consultations are 0.235~0.326 and 0.302~0.419(P<0.01).As a result,we built up an evaluation index system of health education which comprised 3 first-level indexes,19 second-level indexes and 73 third-level indexes for NPC patients undergoing radio-chemotherapy. Conclusion The evaluation index system may provide evidence and a practice tool for clinical nurses in conducting a comprehensive and efficient evaluation of health education among NPC patients undergoing radio-chemotherapy.

Study on the removal of urinary catheter at different times after interventional embolization in patients with unruptured intracranial aneurysm

Objective To investigate the effect of catheter removal at different times on patients with unruptured intracranial aneurysm(UIA) after interventional embolization under the concept of enhanced recovery after surgery. Methods Convenience sampling method was used to select 140 patients with UIA who underwent interventional embolization in an interventional operating room of a tertiary A hospital in Sichuan Province from October 2021 to January 2022 as the research subjects. They were divided into an experimental group and a control group by table method,with 70 patients in each group. After the Aldrete score ≥12 points in the experimental group,the urinary catheter was removed within 2 h after the operation,and the urinary catheter in the control group was removed within 24 h after the operation. Results A total of 135 patients were included in the study,with 68 in the experimental group and 67 in the control group. There was no significant difference in the general data between the 2 groups(P>0.05). The agitation score and agitation rate in the wake-up period before leaving the anesthesia recovery room in the experimental group were lower than those in the control group,and the difference was statistically significant(P=0.013 and P=0.008). The urine output was lower than that of the control group,and the difference was statistically significant(P<0.05).Compared with the control group,there was no significant difference in the first urination in the experimental group(P>0.05). The visual analog scores of pain in the experimental group before,during and 2 hours after the extubation were lower than those in the control group,and the difference was statistically significant(P<0.05). Conclusion Removing the urinary catheter within 2 hours after surgery can help to reduce rate of restlessness during recovery after interventional embolization in patients with UIA,not increase the risk of urinary retention and catheter replacement,and reduce the the pain of the urethra.It is recommended to remove the urinary catheter as soon as possible while ensuring the safety of patients with UIA after interventional embolization.

Modification and effect evaluation of prediction of orogastric tube insertion length in premature infants

Objective To modify the insertion length of orogastric tube placement in premature infants and to evaluate its effect. Methods A total of 306 premature infants undergoing orogastric tube insertion in the Neonatal Center of a tertiary A hospital in Beijing were selected as the study subjects. 155 premature infants undergoing orogastric tube insertion from August 2019 to April 2021 were selected as the test group,and 151 premature infants from March 2018 to July 2019 were in the control group. The effects of different predicting methods on the correct placement rate of the orogastric tube tip,drainage volume before gastrointestinal decompression,number of vomiting episodes,and the total time of gastrointestinal decompression were compared in 2 groups. Results The correct placement rate of orogastric tube tip was 85.81% in the test group and 15.23% in the control group,and the difference was statistically significant(P<0.001). The drainage volume 3 days before gastrointestinal decompression in the test group was higher than that in the control group,and the difference was statistically significant(t=14.38,t=15.78,t=11.06,all,P<0.001). The incidence rate of vomiting during gastrointestinal decompression in the test group was lower than that in the control group(χ2=7.05),and the difference was statistically significant(P=0.008). The total time of gastrointestinal decompression was shorter than that in the control group(t=12.74),and the difference was statistically significant(P<0.001). Conclusion The modified method of predicting insertion length of orogastric tube in premature infants can improve the accuracy of optimal placement for the orogastric tube tip,avoid misplacement of the orogastric tube,and improve the effect of gastrointestinal decompression.

Design and application of loop stoma support rod device

Objective To design a loop stoma support rod device and to explore its application effect in stoma care. Methods The support rod device was self-designed and self-manufactured with 2 fixed wing hard films,2 fixed wing soft films and 1 support rod. Patients who were planned to undergo a loop stoma in a Level A tertiary hospital in Zhejiang Province from August 2019 to October 2020 were enrolled and randomized to 3 groups,namely the experimental group with a self-made loop stoma support rod device,the control group 1 and group 2 with traditional suture method and loop method respectively to fix the support tube. The general information of patients and caregivers,the operation time of nurses and caregivers to change stoma bag,the pain degree during the operation,the incidence of stoma and surroundings complications,and other indicators were compared between the 3 groups. Results There was no significant difference in the general information of patients and caregivers,and the operation time of nurses and caregivers among the 3 groups(P>0.05). The local pain degree scores of patients when nurses and caregivers replaced the stoma bags were(1.90±0.64) points and(1.33±0.83) points in the experimental group,(4.11±0.77)points and(4.21±0.64)points in the control group 1,(2.52±0.68)points and(2.63±0.60) points in the control group 2;there was statistically significant difference between the 3 groups(allP<0.05). The incidences of complications of patients with pressure injury,mucocutaneous separation,and peristomal dermatitis, were 0,3.23%,12.90% in the experimental group, 32.26%,32.26%,54.84% in the control group 1,and 20.00%,26.67%,40.00% in the control group 2;the differences were statistically significant(allP<0.05). Conclusion The self-designed loop stoma support rod device can effectively reduce the incidence of stoma and surroundings complications,reduce the local pain during nursing care,and improve patient’s comfort.

A survey of the position management status for orthopedic specialist nurses

Objective To investigate the current situation of position management of orthopedic specialist nurses(OSN) in China,and to provide references for the formulation of relevant management systems. Methods A self-designed questionnaire was used to facilitate the investigation of the status and satisfaction of the post management of 560 OSN in 28 provinces/municipalities across the country. Results In terms of the management system of OSN,there were only 49.29% of the participants whose hospitals had special management systems for specialist nurses,and 20.89% of the hospital management system was not transparent. Furthermore,the specific rules in employment,level management,performance appraisal,continuing education and training,and post incentive system in those hospitals were different. As for the postpractice of OSN,74.64% of the participants didn’t change their work positions,and their main jobs were still direct clinical nursing practices,but the development of orthopedic advanced nursing practical activities were limited. Job management satisfaction was 56.96%,and only 9.46% had the intention to leave. Most of them(72.86%)were confident about the development and prospects of OSN. Conclusion The satisfaction of post management is at a medium level,as it is at the initial stage,and the advanced nursing practice activities are gradually carried out. The whole team has high professional commitment and considerable development and prospects. It is necessary for the state,hospitals and individuals to work together with their efforts to establish a scientific,rational and perfect system for postmanagement of OSN,so as to improve the growth and strength of OSN team.

Triage management of one-move, two-precise and three-zero during the coronavirus disease 2019 epidemic

Objective To explore a novel triage management mode during the coronavirus disease 2019(COVID-19)epidemic,so as to reduce missed cases,in-hospital cross-infection,and improve satisfaction of patients and their family members. Methods During the COVID-19 epidemic,we moved the triage points to the entries of departments of outpatient,emergency and inpatient by setting up tents. The precise management of triage staff and facilities was enhanced,and the precise triage procedures and information was developed. Results From January 25 to February 20,2020,we triaged a total of 37,698 patients and visitors,among which 301 cases were triaged to the COVID-19 fever clinic,639 cases to the general fever clinic,and 160 cases were suspected as COVID-19 and quarantined,and 3 cases were confirmed as COVID-19 and referred to the designated treatment hospital. Even though the total volume of triage was large,we achieved zero missed detection of suspected patients with COVID-19,zero occurrence of in-hospital cross-infection,and zero complaint from patients and their family members in our hospital. Conclusion The triage management of One-Move and Two-Precise is an effective way and is worth applying to the prevention and control during COVID-19 epidemic or potential infectious disease outbreaks in the future.

Construction of eligibilities and responsibilities for stroke case managers

Objective To establish eligibilities and responsibilities for stroke case managers,and to provide references for training programs and work standards for stroke case managers in accordance with China’s national conditions. Methods Based on literature research,investigation of status of stroke patients management,expert meeting discussions and pre-consultations,a preliminary draft of eligibilities and responsibilities was constructed. Twenty experts from 8 provinces were consulted by Delphi expert consultation method. Results The effective recovery rates of the two rounds of expert consultations were 95.2% and 100%,respectively. In the second round of consultation,the authoritative coefficient of experts was 0.910,and the coordination coefficients of the first,second and third-level indicators of eligibilities and responsibilities were 0.193,0.155,0.141(P<0.005) and 0.445,0.156 and 0.134(P<0.005),respectively. The final eligibilities for stroke case managers included three first-level indicators(basic conditions,professional ability,comprehensive ability),seven second-level indicators and 17 third-level indicators. Responsibilities included four first-level indicators(assessment,planning,implementation,evaluation),17 second-level indicators and 79 third-level indicators. Conclusion The eligibilities and responsibilities of stroke case managers determined by expert consultations are scientific,practical,feasible and innovative,which can provide theoretical basis for nursing practice of stroke case management.

Effects of bundle nutrition intervention on nutritional status and quality of life among elderly patients with gastric cancer undergoing chemotherapy

Objective To explore the effects of bundle nutrition intervention on improving the nutritional status and quality of life among elderly patients with gastric cancer undergoing chemotherapy. Methods A total of 348 elderly patients with gastric cancer who met the inclusion criteria and received informed consent were enrolled and received nutritional risk screening using nutritional risk screening 2002(NRS-2002). Totally 186 patients with nutritional risk were randomly allocated into the experimental group(94 cases) and the control group(92 cases). The experimental group received bundle nutritional intervention. The control group received routine nutrition management and dietary guidance. The body weight,body mass index(BMI),grip strength,lean body mass,body fat,body fat percentage,nutritional intake and quality of life were compared between 2 groups before and after the intervention. Results The prevalence of nutritional risk was 53%. After intervention,the body weight,BMI,grip strength,body fat percentage,lean body mass,75% energy intake and 75% protein intake of the experimental group were higher than those of the control group(P<0.05). The of physical function,role function,emotional function and overall health of the experimental group were significantly higher than those of the control group(P<0.05). The scores of fatigue,nausea and vomiting,loss of appetite and insomnia were significantly lower than those of the control group(P<0.05). Conclusion Bundle nutrition intervention can significantly improve the nutritional status,decrease the prevalence of malnutrition,and improve the quality of life among elderly patients with gastric cancer undergoing chemotherapy.

Nursing cooperation for hybrid recanalization surgery of internal carotid artery occlusion

From nursing cooperation experiences for the hybrid recanalization surgery of internal carotid artery occlusion,key points of nursing cooperation were summarized for the traditional carotid endarterectomy and hybrid interventional surgery:①According to the results of preoperative cerebral angiography,surgery supplies were prepared in advance. ②Equipment in the operating room should be organized rationally to avoid repeated movement of the instrument. ③Patient positioning and care of aseptic area. ④Catheter fixation and care. ⑤Usage of catheters and vascular sheaths for thrombosis prevention and intraoperative cooperation for using anticoagulant.⑥Dose observation for usage of heparin sodium during surgery and additional administration of medicine. ⑦Observation of intraoperative complications and nursing measures. 69 cases were recanalized successful,one case showed recanalization immediately subtraction angiography, one case showed internal carotid cavernous sinus fistula,and the surgery was stopped immediately without causing adverse consequences. All wounds were healed in the first degree.
